We would like to take time to introduce you to our company and the many services that we offer. DCARE HOME HEALTH SERVICES is a full service home health care agency. We provide skilled nursing and therapy services to homebound patient suffering from acte,chronic, or terminal illness; physical or cognitive disability; and other conditions that require skilled nursing and related services that cannet be provided effectively by family or friends. Here is an overview of our services:

Skilled Nursing
A nurse will educate patients on medication and diet regimens,as well as, teaching on the disease process. Each aptient's condition is monitored and reported to their physician.

Certified Nursing Assistant
Our Nursing assistant work under the aupervision of a Registered Nurse. They assist the patient with day-to-day living activities such as personal care needs and light household cleaning.

Medical social worker
The MSW assist with inadequate income to meet medical needs, long term planning and community resource planning.

Home Physician
A physician is offered to the homebound patient. The Physician makes monthly visits as well as PRN visits. The physician works in conjunction with the skilled Nurse.

Therapy

Our agency provides therapy to the patient in the privacy of their home. These therapies include mental, physical, occupational, and speech.. All patients are.

Medical Supplies

DCARE HOME HEALTH SERVICES works in conjunction with our physicians to offer medical supplies to the homebound patient.

Transportation

For patients who need assistance getting to the doctor's appointments DCARE HOME HEALTH SERVICES will provide transportation through our contractors.
